The carpal tunnel is a narrow passageway in the hand in which the median nerve lies. Carpal Tunnel Syndrome is a nerve entrapment syndrome. Carpal tunnel syndrome occurs when the median nerve becomes compressed at the wrist or neck. The median nerve controls sensations to the hand near the palm side of the thumb and fingers. The median nerve supplies the muscles of the hand that move the fingers and thumb. CTS Symptoms can include pain, numbness and tingling of the hand and wrist, weakness, and loss of grip strength. The pain can radiate up or down the arm. Patients with carpal tunnel syndrome often wake up in the middle of the night with numbness and tingling in the hand and arm. If symptoms progress the symptoms will progress to the day. Wasting away of the thenar muscles (fat pad of hand near thumb) occurs in chronic cases. Carpal Tunnel Syndrome is more prevalent in woman due to their carpal tunnel being smaller. Carpal tunnel syndrome is more common in those with repetitive jobs (cashiers), poor biomechanics, trauma, overactive pituitary gland, RA, hypothyroidism, menopause, and pregnancy. Diagnosis for Carpal Tunnel is done through orthopedic (Phalens Test or wrist-flexion test) and neurologic testing. The wrist is examined for warmth, swelling, tenderness, and discoloration. Nerve Conduction Velocity (NCV) and Electromyography (EMG) tests are used to help in the diagnosis.
